1. All weight loss plans start with an initial telehealth assessment of $100. This one-time payment is non-refundable. If a scheduled appointment is missed, you can re-schedule the appointment without paying an additional $100. If a scheduled appointment is missed and not re-scheduled within 14 days of the originally scheduled appointment, it will be considered cancelled and subsequent scheduled appointments will be considered a new appointment and will require the $100 telehealth fee.
2. Weight loss plans will be determined between you and one of our licensed healthcare providers. During the telehealth assessment, the provider may write a prescription for medication to coincide with your treatment plan. Prescriptions will be filled by our pharmacy partner and delivered directly to the address you provide.
3. Follow-up appointments will be scheduled for you by the provider during the initial telehealth assessment. If you need assistance between appointments, please feel free to contact SynergenX Weight Loss by phone or e-mail as listed above.
